The listed pricing would be the reimbursement amount your clinic will receive for each individual device. Email CHAP@familyhearinghelp.org if you need an additional copy of the device and pricing list.
The original manufacturer’s warranty as your office purchases it from the manufacturer applies. CHAP is not able to authorize additional hearing aids until the warranty of the original hearing aid is either expired or used entirely.
Any audiologist, as long they are licensed and listed on the MOU.

An Exhibit B form (found in the executed MOU) will be submitted by the audiologist stating the patient to be fit and with which device. We will then send over an approval or denial for the device. After the patient is fit with their approved device, the provider will send in an Exhibit A form (found in the executed MOU) confirming the patient was fit along with an invoice for the devices.
Once the invoice and all required documentation is received, payment will be submitted to the provider within 60 days.
Sertoma is now reimbursing for services as well as devices. Please include the services provided on the invoice as shown on the service fee schedule. To obtain a copy of the schedule, please contact CHAP staff at CHAP@familyhearinghelp.org or by calling our office at (727) 312-3881.
Both! Any patient a provider has that can take advantage of CHAP is encouraged to have the child apply and can see the approved provider that referred them. Any patients that are eligible for CHAP funding will be paired with a suggested approved provider in their area or they can choose from our list of approved providers.
Sertoma has an approved devices list that can be provided by contacting CHAP staff at CHAP@familyhearinghelp.org or by calling our office at (727) 312-3881. Any suggestions for hearing device models that can be fitted that are not on the approved devices list can be submitted to CHAP staff for approval using the same contact methods.
The practice will handle all ordering and inventory of the devices they are fitting.
Each provider will be offered available patients in their area. It is at the provider's discretion to accept or deny seeing the patient as they see fit.
E-Verify is an internet-based employment eligibility verification administered by the Department of Homeland Security. Data collected on the I-9 form is entered into E-Verify, verifying employment in the United States. The State of Florida requires all providers receiving funding from the state to be processed through E-Verify. As CHAP is funded by the Florida Department of Health, Sertoma is required to verify all subcontractors are utilizing E-Verify.
To find the contractor E-Verify number navigate to the visit the E-Verify website, log-in, and select My Company Account. Your contractor number will be a 7 digit number, consisting of numbers only.
Pursuant to the MOU, patient payments are not allowed under this program.
The maximum reimbursement rate for each product is listed on the approved devices list. The maximum reimbursement rate for each service is listed on the service fee schedule. These costs must be billed directly to Sertoma.
All entities receiving funding from the State of Florida are required to have staff working under the program undergo drug screenigs and background checks. As CHAP is funded by the Florida Department of Health, Sertoma is required to verify subcontractors are receiving drug and background checks.
The expense for the drug screening and background check is required to be paid by the practice through the vendor of their choice.
The program only covers Cochlear Implant processors at this time.
While Sertoma does not handle insurance through CHAP, insurance coverage will be applied to the device's cost and the CHAP funding will cover the difference.
If your practice is in-network with an insurance company a patient has, CHAP will pay any expenses the family would ordinarily pay relating to the insurance, like a co-pay, for example. Patients will continue to have no out of pocket expenses, even when using insurance.
