Children's Hearing Aid Program:
Information for Audiologists!
How It Works
1. Become an approved Provider
Work with one of our patient advocates who will guide you through the MOU process.
2. Patient applies for CHAP
Your office will refer patients who may qualify to complete an application. You will be notified of the patient's outcome.
Additionally, Sertoma may refer patients to your office who apply and do not already have an audiologist.
3. Request for Authorization
A request for authorization must be submitted to Sertoma requesting the device(s) the patient requests. Sertoma will return the authorization within 48 hours.
4. Fitting
The audiologist will order a new hearing aid, or fit the child with a hearing aid from the practice's stock.
5. Invoicing & Reimbursement
The practice will send a bill to Sertoma for the amount previously authorized, along with a document stating exactly what device the patient was fit with.
Payment will be remitted to the office after the invoice has been processed.
